Decentralised identity
A participant creates and holds their own identity, so the wellness record is theirs rather than an account inside somebody else's system.
Implementations HealthTech
FINAO's Wellness Café is a blockchain platform for chronic-illness prevention: decentralised identity, activity tracking, tokenised rewards and AI-driven personalisation, piloted with the YMCA across the Pacific Northwest.

Overview
FINAO builds on a blunt statistic: around 80% of chronic illness is preventable. Prevention, though, depends on sustained everyday behaviour, and everyday behaviour generates exactly the kind of personal health data people are least willing to hand over.
Wellness Café addresses both halves at once. Participants create a decentralised identity they control, log activity against it, and earn tokenised rewards for doing so, so the record that makes personalisation possible stays in the participant's custody rather than accruing to a platform.
The programme frames wellness across eight dimensions: physical, spiritual, financial, occupational, environmental, emotional, social and educational, and runs in both virtual and in-person modes, for individuals, employers, schools and government health programmes.
A P0 beta is live with the YMCA and its members across the Pacific Northwest, which is where the model meets real participants rather than a design assumption.
What it uses the graph for
01
A participant creates and holds their own identity, so the wellness record is theirs rather than an account inside somebody else's system.
02
Virtual and in-person activity is logged against that identity, producing the data personalisation needs without moving custody of it.
03
Incentives are issued as tokens on Rubix, which makes participation portable and the reward ledger independently checkable.
04
Guidance adapts across the eight wellness dimensions, driven by data the participant has chosen to make available.
